Giving you $10 credit as an inconvience on overpriced, low quality crap is an insult. Not a big deal since the customization is god awful from the "offical shop" of the NHL. dsl135 Posted June 3, 2014 Report Share Posted June 3, 2014 (edited) For anyone who is planning on sending their jersey to Bobcat Athletic and needs the order form to send along with their jersey, I received a response from them a week or so ago and have the order form in PDF form. Just let me know and I can forward you the PDF. Note, this probably won't help with ordering kits. Its pretty specifically an order form for sending in with your jersey.
